Demetri is introduced in the side story IL Siracusano. He is a high-ranking member of Famiglia Bellone, whose territory is a city known as Volsinii. He is the personal soldier of Don Bernardo Bellone's son Leontuzzo Bellone.
Volsinii is aflutter with activity at the start of Siracusano, because a new core is being affixed to a static city within Volsinii known as Cittanova, which would turn it into a nomadic, moveable city. Rumour is that the new plate will be run by Famiglia Bellone, as Leontuzzo Bellone is working at il Ministero dei Lavori Pubblici as a secretary to Ministro Carracci. However, chaos around the city has been erupting gradually and the crime rate has gone through the roof. Since the city belongs to Famiglia Bellone, it is "obvious" somebody is trying to pick a fight with them.
In IS-ST-1, a clerk of Leontuzzo's is targeted by hitmen. He runs for help and manages to make it to Lavinia Falcone, a judge on her way to work. The hitmen flee, only to run into Capone and Gambino, two hitmen specifically hired by Demetri in order to silence them. The hitmen are killed, and Demetri goes to meet Capone and Gambino personally. He brings with him a number of mafiosi, and gives Capone and Gambino a task to leave the altercation alive, and he will reward them. He tells them he won't fight them himself, or they'd just be killed by him and it'd be unfair.
Later, Demetri receives a call from Lavinia, who locates the bodies of the hitmen. She wants to know if he knows of any hitmen prowling around the city lately. He feeds her some lies that nobody would dare to pull a reckless stunt in the territory of Famiglia Bellone, therefore the mastermind would need to be somebody not of Siracusa, or somebody who isn't afraid of the retaliation of Famiglia Bellone. She asks him to keep her updates if he hears anything, and he tells her to fuck off. She reminds him she is important to the famiglia, and he concedes that he will tell Leontuzzo if he learns anything.
In the meantime, Leontuzzo asks his bodyguard Cellinia Texas to protect Ministro Carracci from assassination while the two of them attend a banquet. However she is distracted by Lappland Saluzzo, and a car bomb goes off, killing Carracci. At the same time, assassins enter the banquet venue in order to kill Leontuzzo. They are scared off by Cellinia, but Leontuzzo is injured. Lavinia arrives to take Cellinia into custody as a witness to the murder of Carracci, but takes Leontuzzo back to the Bellone Estate on the way.
At the estate, Demetri is already present to accept him. A doctor assesses Leontuzzo's injuries, and Demetri peels fruit for Leontuzzo to eat once he wakes up. He tells Leontuzzo that the famiglia have already killed the assassins. Leontuzzo speculates Columbian infighting as being the reason for the assassination attempt, and asks Demetri for some water. Demetri pours him a cup of boiling water because he is annoyed with him for letting Lavinia take Texas. He thinks it makes Leontuzzo look weak, in addition to being injured, at a time when the Famiglia already have egg on their faces. Leontuzzo tells Demetri that he thinks the hit is coming from inside their own famiglia, and he wants them to think that Famiglia Bellone is in disarray. He thinks that the hits will become more reckless without Cellinia present and tells Demetri to keep an eye out for the true culprit.
Instead, Demetri arranges for the carbombing culprit to be tied up and stuffed into a garbage can in Cittanova park. It's a trap in order to assassinate Lavinia. One of the mafiosi tells Demetri he feels bad for Lavinia, and Demetri reminds them that she has gotten too big of a head, and she has made Leontuzzo soft with her fantasy ideals of a Siracusa that runs on "law" and "order". Her assassination is crucial to send a message to Leontuzzo, because they need Leontuzzo to be ruthless and focused to seize victory from the looming chaos. The mafiosi are concerned Leontuzzo will blame them, and Demetri promises that he will take Leontuzzo's ire himself.
Lavinia recognises the assassin as a former fellow judge, but the assassination is foiled when Cellinia arrives. Demetri is satisfied that seeing her coworker fall to the mafia ways is bound to have crushed Lavinia's spirit, and that might be enough to wake up Leontuzzo. He orders all the mafiosi nearby to fall back before they are killed by Cellinia.
